Women’s Hall of Fame Nomination Form

Nominees may be living or deceased. Previous inductees are not eligible. Past Commission for Women members may be nominated after two years of being off the Commission. Nominees that are not selected may be resubmitted for consideration the following year. Nominations are due October 31st for the next year’s event. During the selection process the selection committee may decide on a different category than submitted when reviewing all information received.

The selection committee scores each nomination using a Scoring Rubric. Review the Rubric before completing the Nomination Form. If you prefer to complete the form offline, download the Nomination Form in English or Nomination Form in Spanish.
